If you choose to email your application and supporting documents we ask that you properly scan all materials, cell phone photos are not accepted.
Incomplete applications will not be considered and returned to applicant for completion. Please note all REQUIRED fields on your application.
For all Personal Equipment applications we require that you submit a personalized quote from an equipment vendor. This quote should include include cost and specifications for your individual child. (Screenshots or general quotes from internet searches are not accepted). The reason we ask for this is because the price of the equipment varies depending upon the unique needs and measurements for each child. Personal Equipment applications submitted without a vendor quote will be considered incomplete.
Approved applicants can receive up to 12 hours of provided Respite Care per month, at no cost to the parent/guardian.
Yes, for all Hearing Aid applications we require that you submit a current audiogram (within past 12 months).
For all hearing aid applications we require that you provide a letter from the child’s audiologist recommending the use of a Oticon hearing instrument.
The letter must state if a brand other than Oticon is requested, and provide justification of such request.
Teachers, counselors, aids, or other advocates may assist in the completion of an application, but all applications do require the consent of a parent/guardian. Personal information pertaining to the parent/guardian such as Income and Expense Verification must also be provided.
