COVID-19 Relief

If your family that cares for a special needs child (or children) has been adversely impacted by the corona virus pandemic and would like to request assistance with diapers, bed pads, formula, feeding tubes, or other commodities necessary for the care of your child, please fill out this application as thoroughly as possible. Please note that we cannot provide direct monetary support for rent, utilities, or delinquent bills.  All responses must be printed legibly in English.

Once the application is filled out, you may save it and send by email along with supporting documents (preferred). Or, send everything via a quality fax machine. Or, print out all materials and send via USPS. Photos of documents are not acceptable. Incomplete applications will not be considered and returned to applicant for completion.
